There are systems and methods for preparing or using prognostic information about opioid response. The information may include whether subject has a subject genotype that includes a COMT haplotype diploid, at least two SNP diploids, one or more demographic phenotypes or a combination thereof. The COMT haplotype diploid is a combination of two COMT haplotypes selected from an LPS haplotype, an APS haplotype, a HPS haplotype or a combination thereof in the COMT gene.
