Abstract:
Apparatus for remodeling in the urinary tract of a human female comprising a remodeling device having a shaft with a profile and with proximal and distal extremities. A handle is secured to the proximal extremity of the shaft. A member is mounted on the distal extremity of the shaft. A plurality of needle electrodes are carried by the distal extremity of the shaft and are disposed circumferentially of the shaft. A pusher is carried by the handle for moving the needle electrodes from a retracted position in which they are within the profile of the shaft and an extended position in which they extend sidewise beyond the profile of the shaft. A pump is connected to the handle for supplying a cooling liquid to the distal extremity of the shaft so that it exits from the shaft in the vicinity of the needle electrodes. An RF generator is coupled to the handle and to the needle electrodes for supplying radio frequency energy to the needle electrodes.

Abstract:
A method and apparatus for inflating a balloon at a distal portion of an elongated delivery catheter to contact surrounding biological tissue, and expanding a refrigerant adjacent the balloon to cool the biological tissue to render it non-viable. The invention provides systems containing a sterile container for aspiration, filtering, treating and reinjection into a subject of a bodily fluid in a sterile environment. The systems are particularly designed to facilitate transfection of cells within the sterile container by gene therapy molecules and reinjection of the transfected cells into the donor at controlled depth in precisely controlled volumes such as is useful for revascularization of ischemic cardiac tissue. For injection, a pressure actuator, preferably hand-held, applies force to liquids held within the sterile container in precisely controlled increments, thereby expressing liquids through a hollow needle or injection catheter in precisely controlled microvolumes. The invention systems include a hand-held injection device with audible cues that correspond to an operator-selected injection volume and/or audible cues that correspond to needle penetration depth selected by the operator.
Balloon catheters according to the present invention have surface contours with protrusions and grooves to make non-continuous lesions. There is a heating element inside the balloon. The balloon will be filled with a fluid before ablation to expand the balloon for tissue contact, and to transfer heating energy from the heating element to the tissue in contact with the balloon. Since the balloon surface has contours with grooves and protrusions, the tissue contact will not be continuous along the balloon surfaces, and therefore the ablation on the tissue will not be continuous. The energy transmitter can be ultrasound transducer, electrical resistant heater, and microwave antenna.

A method and apparatus for using a secondary refrigerant to precool and liquefy a primary refrigerant, then vaporizing and expanding the primary refrigerant to cool a cold tip of a cryosurgical instrument for ablation of biological tissue, such as cardiovascular tissue, in particular endocardiac tissue and tissue inside a cardiac blood vessel. The secondary refrigerant has a critical temperature above the critical temperature of the primary refrigerant, and a cooling temperature below the critical temperature of the primary refrigerant, thereby facilitating the use of the precooling step to provide liquid primary refrigerant in an operating room environment in which the primary refrigerant could not otherwise be provided in the liquid phase.
