Abstract:
The present invention provides methods and an apparatus for treating patients having ulcerations of the lower foot, including patients suffering from diabetic neuropathy. The brace generally includes a pair of rigid shells, a fastening system for joining the shells together to form a unified brace and a combination of bladders which engage the patients leg and ankle to off-weight the plantar surface, prevent plantar flexing and minimize shearing forces to the plantar surface. The volume of the bladders may be adjusted by the addition or removal of a filler in order to maintain a uniform pressure between the bladders and the patients leg. The filler contained in the bladders preferably does not undergo significant expansion or contraction in response to temperature or altitude changes. The brace also preferably includes a series of full-length bladders and a specially designed adjustable bladder located within the ankle and heel region of the brace to maximize off-weighting and to prevent the foot from moving downward toward the footbed of the brace. In addition to providing an effective means for off-weighting the plantar surface of a patient, the brace is reuseable, easy to apply and fully adjustable to accommodate changes in a patients leg size.

Abstract:
An improved apparatus and method for applying a total contact cast to a patient utilizes a kit of pre-packaged materials containing all of the items necessary. A uniquely designed padding strip pads the bony prominence on either side of the ankle, the top metatarsal area, and the crest of the shinbone in one unitary piece. Special cuts in the padding strip allow it to conform tightly to the bend between the foot and lower leg. The special cuts also allow the connected circular or elliptical flaps to also conform easily to the bony prominence of the ankle. Positioning the padding strip is simpler, easier, faster, and provides greater structural integrity to the overall cast. The unitary design helps prevent the padding strip from shifting when the cast is complete. An enlarged base portion of the walking heel eliminates the need for a separate wood platform for support.
Embodiments of the present invention provide a system for limb off-weighting, including a sleeve operable to wrap at least partially around a leg, a support strut coupled to the sleeve to receive weight applied to the sleeve, and a foot assembly configured to support and protect the bottom of a foot, wherein the foot assembly is coupled to the support strut and operable to receive weight applied to the sleeve and transfer the weight to underlying surfaces, thus off-weighting and reducing pressure and shearing forces on the foot and ankle. According to some embodiments, foot assembly pivots around the support strut providing an assembled mode and a storage mode. According to other embodiments, sleeve is an open-ended conical shape. According to various other embodiments, the support strut may be tilted to accommodate legs and/or calves of varying sizes, and the foot assembly may adjust to feet of varying sizes.

Abstract:
Embodiments of the present invention include an apparatus for encouraging compliance with a prescribed orthotic treatment, including an orthotic device applied to a limb of a user, a strap coupled to the orthotic device, the strap having two ends and configured to wrap around the orthotic device at an attachment area, where a length along the strap is smaller than a maximum circumference of the limb distal to the attachment area, and a locking mechanism to lock the strap onto itself. Other embodiments of the present invention include a system for off-weighting a user's foot, including a hardenable cast; a rigid outer boot having a footbed, a first side strut with a first strap aperture, a second side strut with a second strap aperture, and a strap extending through the first strap aperture and the second strap aperture and operable to tighten around the hardenable cast.
