Abstract:

Disclosed are methods and apparatus for the detection of inadvertent endobronchial intubation, esophageal intubation and other potentially dangerous complications arising from invasive monitoring, endotracheal intubation and mechanical ventilation during surgery. Methods of the present invention generally require producing a mixed electrial signal comprising the combination of a first electrical signal and a second electrical signal, the first electrical signal being representative of the sounds emanating from a first region of the patients body and the second electrical signal being representative of the sounds emanating from a second region of the patients body. The mixed electrical signal is converted into a display signal, and then the display signal is monitored while reducing the contribution of said first electrical signal in said mixed signal to a value substantialy less than the contribution of said second signal in said mixed signal. Apparatus comprising at least two electronic sound sensing devices are disclosed. Means for independently and adjustably amplifying the electrical signals are provided.

Abstract:

An implantable device for sensing in vivo the level of at least one blood constituent in mammalian vascular tissue has at least one source of radiation from infrared through visible light, arranged to direct the radiation at the tissue where it is affected by interaction with the tissue, and a plurality of detectors. Each detector has associated with it a filter transparent to a discrete narrow band of radiation, and each detector provides an output signal representative of detected radiation in that narrow band. The detectors are located with respect to the tissue to receive radiation affected by the tissue. The invention also encompasses a device for measuring and controlling the level of a blood constituent, such as glucose, in a mammal, and includes an implantable infrared source and sensor module for directing infrared radiation through vascular tissue and for sensing the infrared radiation after it has passed through the tissue and generating an output signal representative of the sensed infrared radiation. The device includes a processor module responsive to the output signal from the infrared source and sensor module which performs spectral analysis of the output signal and derives from that analysis a control signal representative of the level of the blood constituent. The device further includes an insulin pump for dispensing doses of insulin in response to the control signal for blood glucose regulation.

Abstract:

This invention is an implantable sensor and system capable of measuring, controlling, monitoring and reporting blood constituent levels. An integrated system providing a mechanical and chemical barrier against the spread of infected secretions into the distal trachea is disclosed. An endotracheal tube is used for patient airway management and provides a means to conveniently irrigate and drain the subglottic region below the vocal cords and above an inflated cuff. The subglottic region accumulates liquid secretions that may channel past an inflated endotracheal tube cuff, providing the necessary bacterial inoculum leading to bronchitis and nosocomial pneumonia. An irrigation channel delivers liquids such as saline or antibiotic and antifungal medications for mucosal hydration, and bactericidal action against infected subglottic secretions. An outer sleeve surrounding the endotracheal tube forms a suction lumen for removing the secretions. Electronic and mechanical controls provide regulated volume infusion and regulated suction.
