Creation and population of a database of personal health care information using subjective data, health care behavior, and medical records A particular embodiment relates to the combination of data in the form of a personal health profile from the profile subjects and third parties holding such data. Key data is extracted from the clinical medical record of the profile subject to create a first dataset that can be included in a personal health profile. The personal perceptions of clinical interactions of the profile subject with health care practitioners and medical care delivery systems is a second dataset that can be included in a personal health profile. The health care behavior outside of medical care delivery systems of the profile subject is a third dataset that can be included in a personal health profile. In a preferred embodiment, all three datasets are combined. In other embodiments, any two of the three datasets could be combined to create unique personal health profiles. The resultant collection of data is stored as a personal health profile. The datasets can be updated on a periodic basis or upon an event, such as a doctor visit. In a preferred embodiment, an updated profile is generated using the existing personal health care profile and the updated data provided. In some embodiments of the present invention, volunteers, profile subjects, and third party record holders are compensated for their participation.
