Abstract:

A surgical clip applicator system for receiving a clip from a clip holder and for compressively applying the clip to a body vessel or tissue comprises a stationary front jaw member, a rotatable rear jaw member, and rear jaw member actuator. The stationary front jaw member includes a rear facing grooved surface and a rear facing depressed notch above the grooved surface which further includes a rearward projection along the upper part of the notch. The rotatable rear jaw member includes a front facing grooved surface, a front facing depressed notch above the grooved surface which further includes a frontward projection along the upper part of the notch, a first location wherein the actuator is rotatably connected to the rear jaw member, and a second location wherein the rear jaw member is rotatably connected to the front jaw member. The V-shaped open clip is manually snapped onto the clip holder and is thereafter snapped onto the front and rear facing grooved surfaces of the surgical clip applicator for preventing lateral motion of the open clip and wherein the frontward and rearward projections prevent upward movement of the open clip. The actuator causes forward rotation of the rear jaw member onto the jaw member thereby compressing the clip onto the vessel or tissue.

Abstract:

A surgical instrument for dilating blood vessels comprises: a front multilinkage mechanism; an intermediate hollow cable-wire structure combination; and a rear manually operated control mechanism. The rear manually operated control mechanism further comprises: a rearmost thumb ring being connected to a stationary member; and index finger and ring finger rings being connected to a longitudinally movable member. Motion of the index finger and the ring finger rings relative to the thumb ring causes similar motion of a wire which is located in the hollow cable. The front multilinkage mechanism further comprises: a plurality of longitudinal ribs being located on a rear stationary section; a similar plurality of rear linkages; a similar plurality of front linkages; and a similar plurality of longitudinal ribs being located on a movable front section. The front end of the wire runs through the rear stationary section to the front stationary section and has attached thereto a spherical ball adjacent the rear linkages. The frontmost end of the wire inserts into and moves freely within the front stationary section.

Abstract:

This invention relates to surgical instruments used, for example, in approximation of the sternum after median sternotomy. The device according to this invention is utilized to tighten applied sternal sutures, such as wires, by first winding up the sutures on a spool, then twisting the sutures for fixation and to provide an appropriate final tension. A cutting mechanism is included for severing the sutures so as to leave a short twisted segment, resulting in a joint secure against slippage. Waste sutures are stripped from the spool against a stripping plate by axial translation of the spool. A crank and sleeve may be included to facilitate twisting the sutures for fixation and final tensioning. An optional ratchet or similar mechanism may be used to prevent unwinding of the sutures from the spool during operation.
