Abstract:

A system which facilitates the closure of puncture wounds in the vasculature of a patient by helping locate and isolate the site of the puncture wound in the patient. A tube or catheter having an indicator hole for aspirating blood assists in locating the exact site of the wound. A retractor moves the surrounding tissue laterally forming an access path to the wound, and acts as a guide for the wound closure device. The retractor and accessories are preferably used in combination with a surgical clip applicator which delivers clips to the site of the wound, but can also be used with other methods of wound closure such as suturing and stapling.

An improved device and method are provided for securing the urethra to the bladder in surgery. A trocar is provided for insertion into the urethra. The trocar is removably secured to the urethra for advancement of the proximal end toward the bladder. A sheath is inserted and secured to the bladder in order to assist in the advance of the bladder toward the urethra. The improved system permits medical personnel to clamp the urethra and bladder without the need for insertion of time consuming and laborious sutures. A method of securing the urethra and bladder using the disclosed apparatus is also provided.

Abstract:

A triceps for anastomosis, the triceps having two outer legs on opposing sides of a middle leg. A latch is located on one outer leg and the middle leg to hold the respective legs in a clamped position. An alignment post extends through the middle leg and cooperates with holes in the outer legs to allow the post to enter the holes and ensure alignment of the legs in use. The post has an enlarged base portion that does not pass through the hole and thereby limits movement of the outer legs relative to the inner legs and thereby limits clamping pressure at the free ends of the legs. The free ends of the legs have various cooperating shapes to facilitate everting and aligning tubular vessels and body tissue for anastomosis.

Abstract:

An apparatus for facilitating the locating, everting, and closing an opening in a blood vessel. The apparatus can include a closure instrument having an elongated member defining a longitudinal axis and proximal and distal ends. The elongated member has a vacuum lumen extending at least a portion of the length thereof for conveying a vacuum and terminating in a vacuum port adjacent the distal end of the elongated member. The distal end of the elongated member is dimensioned to be positioned proximal a vessel opening in a blood vessel whereby vessel edge portions defining the vessel opening are at least drawn toward the vacuum port in response to a vacuum conveyed through the vacuum lumen. At least one surgical clip is mounted adjacent the distal end of the elongated member and is adapted to be formed to an at least partially formed condition thereof. The one clip is positioned with respect to the vacuum port to engage the vessel edge portions drawn toward the vacuum port upon movement of the one clip to the formed condition thereof to generally approximate the vessel edge portions to at least partially close the vessel opening. A method for locating, everting, and closing an opening in a blood vessel is also disclosed.

Abstract:

A patch applicator has an elongate body with an inner lumen and an outer lumen arranged concentrically. A patch is also provided. In operation, a source of vacuum draws a vacuum through the inner lumen. The inner lumen is applied to a patch to releasably hold the patch. The patch is advanced within the patient until it is placed on top of and over a wound in tissue. Flowable adhesive is injected through the outer lumen onto the patch and tissue surrounding the patch. The patch applicator holds the patch in place, allowing the adhesive to at least partially set. The applicator is then removed from the patch. Thus, an adhesive patch is attached to close an opening in tissue.
