Abstract:

The present invention relates to methods and apparatus for detection and treatment of syncope in an implantable medical device, and particularly to detection of syncope as a function of a predetermined increase in one or more respiration parameter and drop in heart rate and optionally delivering a pacing therapy in response thereto. The onset of a syncopal episode is declared when the patient's respiration rate and/or tidal volume and/or minute ventilation increases by a predetermined increment or threshold and a heart rate drops below a threshold heart rate drop. The threshold heart rate drop is preferably established as a function of the change in the respiration parameter.

Abstract:

Oxygen saturation data is monitored during a predefined window to obtain a measurement of circulation delay. The measured circulation delay is used as a basis for determining therapies, including overdrive pacing. In some embodiments, circulation delay is used to identify patients that will benefit from overdrive pacing as a therapy for sleep disordered breathing.

Abstract:

A method of determining a respiration parameter in a medical device in which pressure signals are sensed to generate corresponding sample points, and a breath detection threshold is continuously adjusted in response to the generated sample points to generate a current adjusted breath detection threshold. A current generated sample point is compared to the current adjusted breath detection threshold, and the continuous adjusting of the breath detection threshold is suspended and the breath detection threshold is equal to the most current adjusted breath detection threshold generated prior to the suspending in response to the comparing. A next sample point, generated subsequent to the suspending, is compared to the set breath detection threshold, and the respiration parameter is determined in response to the comparing of a next sample point to the set breath detection threshold.

Abstract:

An apparatus for determining a respiration parameter in a medical device in which a pressure sensor senses pressure signals, and a signal processor, coupled to the pressure sensor, receives the sensed pressure signals and generates corresponding sample points. A microprocessor continuously adjusts a breath detection threshold in response to the generated sample points to generate a current adjusted breath detection threshold, compares a current generated sample point to the current adjusted breath detection threshold, suspends the continuous adjusting of the breath detection threshold and sets the breath detection threshold equal to the most current adjusted breath detection threshold generated prior to the suspending, and determines the respiration parameter in response to a comparing of a next generated sample point to the set breath detection threshold

Abstract:

A method of and apparatus for determining the physical posture of a patient's body, having a superior-inferior body axis, an anterior-posterior body axis and a lateral-medial body axis, in relation to earth's gravitational field. A medical device having first, second and, optionally, third accelerometers having sensitive axes mounted orthogonally within an implantable housing is adapted to be implanted with the sensitive axes nominally aligned with ideal X, Y and Z device axes correlated to patient body axes. Each accelerometer generates DC accelerometer signals having characteristic magnitudes and polarities on alignment of the sensitive axis with, against or normal to earth's gravitational field and DC accelerometer signals of varying magnitudes and polarities when not so aligned. The actual pitch and roll angles of the sensitive axes of the accelerometers in the implanted IMD with respect to the true gravitational axes are determined and the yaw angles are determined or estimated. Corrected DC accelerometer output signals are derived from the pitch, roll and yaw angles and are compared to a set of stored thresholds for each body posture to determine the actual body posture.

Abstract:

An intracardiac ventricular pacemaker is configured to detect a ventricular diastolic event from a motion signal received by a pacemaker control circuit from a motion sensor. The control circuit starts an atrial refractory period having an expiration time set based on a time of the detection of the ventricular diastolic event. The control circuit detects an atrial systolic event from the motion signal after expiration of the atrial refractory period and controls a pulse generator of the pacemaker to deliver a pacing pulse to a ventricle of a patient's heart at a first atrioventricular pacing time interval after the atrial systolic event detection.
